
ICARUS
An immunoprevention trial concept for patients at high risk for recurrent cutaneous squamous cell carcinoma.
The clinical problem
A subset of patients with cutaneous squamous cell carcinoma develop repeated new tumors over time and remain at substantial risk despite treatment of each individual lesion.
ICARUS was developed around a different therapeutic question: rather than waiting for each subsequent tumor to emerge, could immunotherapy be used proactively to reduce the rate of future cutaneous squamous cell carcinomas in a high-risk population?
Central Question
Can systemic immunotherapy be used as immunoprevention to reduce the burden of future cutaneous squamous cell carcinomas in patients at particularly high risk?
Questions explored
Who is high risk?
How should a population with sufficiently high baseline CSCC incidence be defined so that a prevention strategy can be studied meaningfully?
What should prevention mean?
Should success be measured by fewer tumors, a lower rate of tumor development, delayed recurrence, or another clinically meaningful endpoint?
The ICARUS concept
The Research Forum session reviewed the hypothesis, scientific rationale, and proposed schema for ICARUS.
The central concept was to evaluate immunotherapy not as treatment for an existing advanced cancer, but as a preventive strategy in patients with a demonstrated propensity to develop repeated high-risk cutaneous squamous cell carcinomas.
This framing shifts the therapeutic objective from treating an individual tumor to altering the patient’s future disease trajectory.
Why It Matters
For patients who repeatedly develop consequential CSCCs, the relevant clinical burden may be the ongoing rate of new cancers rather than any single lesion.
Research Forum focus
The presentation was structured around early trial development: the biologic rationale for immunoprevention, identification of an appropriate high-risk population, selection of clinically meaningful endpoints, and construction of a feasible prospective study.
The Forum provided an opportunity to challenge the assumptions behind the concept before the trial design was fixed.
